To facilitate the application process for students residing outside Sulaimani, including all high school graduates (scientific and literary branches), we would like to inform you that it is not necessary to visit the institute in person to obtain the application form; instead, you may download it directly.

Please follow the steps below to complete the form:

1. Print the form double-sided on one A4 sheet of paper.

2. Complete the form using a blue pen.

3. Provide your personal information and affix a recent color photograph.

4. Take the form to your high school for verification of grades and the required signatures.

5. Submit the form to the Education Department of your city or town to obtain the official stamp and signature.

6. Return the completed form to the institute and, upon submission, pay the application fee of 4,000 IQD (four thousand dinars).

Important Notice:
This form is strictly for students applying from outside Sulaimani. Since the application form used within Sulaimani differs, graduates residing in Sulaimani must collect their forms directly from the institute.
